We aim to make practice information and services usable by people with disabilities. We work to improve readable content, clear navigation, keyboard access, text alternatives, and usable forms. This statement is not a claim that every page or outside service has been independently certified as accessible.
Request assistance
If you encounter a barrier or need information in an accessible format or help with a communication method, call (312) 426-2705 or email info@dolomitepsych.com. Tell us the page or service, the difficulty you experienced, the assistance you need, and a safe way to reach you. Do not include detailed health information in ordinary email.
We will work with you to identify an effective way to provide access, including an alternate format or communication method when appropriate. A patient's request for assistance is separate from consent to SMS or any clinical treatment choice.
External services and urgent needs
Patient portals and other outside services may use different technology. Tell us about a barrier so we can help arrange an appropriate alternative and report the problem to the provider.
Accessibility requests and website messages are not emergency channels. For an immediate medical or safety emergency, call 911 or go to the nearest emergency department. Call or text 988 for the Suicide & Crisis Lifeline when appropriate.
